Teachers
The New York State Education Department (NYSED) outlines certification requirements for teachers. It is the responsibility of every teacher to keep their certification valid and up-to-date.
Teachers must hold a valid and appropriate NYSED teacher certification to teach in New York State public schools.
Certain changes related to your teacher certification must be reported to NYSED and HR. These changes typically include:
Name and Address Changes: If you legally change your name, you must notify Human Resources and update your certification records.
Instructions on how to change your name or address (See Note 1 below):
(i) TEACH Account (name change)
(ii) TEACH Account (address change)
(iii) NYSED Name Change Form (name and address change)
Employment Status: Changes in your employment, such as starting or leaving a teaching position, may need to be reported.
Disciplinary Actions or Legal Issues: Any actions that might affect your certification status, such as disciplinary measures or legal matters, must be disclosed.

Note 1: Additional considerations for Name and/or Address changes.
Report name or address changes to the New York State Teachers Retirement System (NYSTRS) - if you are a member, by completing and mailing to them the Name/Address Change Form (3 weeks to take effect) or by calling them at 1-800-356-3128.
Report name or address changes to the New York State and Local Retirement System (NYSLRS) – if you are a member, by completing and mailing to them the Name Change Form and/or the Change of Address Form or by calling them at 1-800-805-0990.
Report name or address changes to your union representative – if you are part of a bargaining unit and a member.
Instructional Personnel only (Teachers and Teaching Assistants): Report address changes to New York State United Teachers (NYSUT) by updating it in your NYSUT account or by calling (518) 213-6000.
Report name changes to the Social Security Administration office by filing out the SS-5 Form and returning it to your local Social Security Office. For address changes login to your SSA online account or call 1-800-772-1213.
Teaching Assistant
Teaching assistants must either be certified New York State Department of Education (NYSED) teachers or teaching assistants.

Certain changes related to your teaching assistant certification must be reported to NYSED. Refer to the instructions above on how to change your name and/or address.
